The ACE Program Application & Student Enrollment Enrollment (please read)
Once accepted, this enrollment process must be completed prior to requesting and booking student services. The enrollment process consists of the following reading and agreeing to these items: Service Provider Guidelines; Parent/Guardian consent statement, Hold Harmless Waiver and Student Handbook Acknowledgement Form. Upon submitting this packet, the student may view their New Student Orientation Video (optional) or schedule and appointment with the SSC on the ACE website. If the enrollment process has not been completed, the student’s enrollment will be placed on hold or added to the waiting list (if applicable) and other students will be admitted into the program in their place.
Service Provider Guidelines (please read):
If a student wishes to attend a service the request must be made using the myACE Store. If the service has reached capacity, the student and parent/guardian will be notified if a cancellation occurs. The student will be able to schedule and attend a service on the same day only if a cancellation occurs or as permitted by the service provider.
On-site/in-person services will be scheduled more frequently as the pandemic subsides. Information about each service is provided in the myACE store and students along with the parent/guardian will be required maintain service/activity reminders. Your myACE portal will conveniently store all your booking information to review at any time.
The program is unable to provide separate staffing for on-site services. Parent/guardians must make adequate plans to accommodate their child’s needs prior to the scheduled service. In addition, it is understood that when a service is scheduled, the student must be able to participate as expected and agrees to maintain good behavior and follow guidelines and procedures set forth by the service provider.
Authorization of Consent to Participate in Student Services on behalf of a MINOR:
(I)(We), do hereby consent to my/our child, a minor, participating in student services conducted by affiliated service providers of the ACE Program. I also consent to release my child’s grades and/or progress reports to the ACE Program to monitor progress for student academic services.
It is understood that this authorization is given in advance of any specific service received but is given to provide authority and power on the part of our aforesaid student to give specific consent to participate in services as selected by the student. This authorization shall remain effective through the 30th day of September of the end of the grant year, unless sooner terminated in writing.
Hold Harmless Waiver Form (please read):
I give permission for my child to attend student services that are selected for the purpose of education, career training, or mental health assistance. I understand that these services may take place at businesses, on college campuses, museums, and a variety of locations for educational purposes. I give permission for said child to participate in activities that are geared for my child away from the supervision of the American Indian Center of Arkansas or any authorized employees of the ACE Program.
I further agree to assume all responsibility for any liability, which may arise out of ordinary negligence or otherwise, with the exception of acts of gross negligence, or intentional, willful, or wanton misconduct of the service provider.
On behalf of myself, my heirs, assigns, child, and next of kin, I waive all claims for damages or injuries sustained to my child or property that may have occurred in route to, at, or from such activity whether caused by the ordinary negligence of the released party or otherwise, with the exception of acts of gross negligence, or intentional, willful, or wanton misconduct as indicated above.

I understand that if any acts of gross negligence, or intentional, willful, or wanton misconduct should occur as indicated above, the American Indian Center of Arkansas or any authorized employees of the ACE Program will not be held liable for such damages or injuries.
